What is included in the evaluation fee?
The fee covers your complete intake review, clinical evaluation by a licensed provider, and documentation if clinically appropriate. There are no hidden fees.
When do I pay?
Payment is collected at the time of intake submission. You will be charged once you complete the intake form and submit for evaluation.
What if documentation is not issued?
Documentation is issued solely based on the licensed provider's clinical judgment. The $99 fee compensates the clinician for the evaluation itself, regardless of outcome, and is non-refundable. See our refund policy for full details.
Do you accept insurance?
At this time, evaluations are self-pay. We do not bill insurance for ESA evaluations.
